Role Description
The Consultant Pharmacist is a full-time hybrid role based in Peoria, IL, with some work-from-home flexibility. Day-to-day responsibilities include conducting comprehensive medication regimen reviews, ensuring regulatory and clinical compliance in long-term care and other pharmacy settings, and documenting findings and recommendations in accordance with applicable standards. The role involves collaborating with facility staff and prescribers, providing education on medication management, and supporting quality improvement initiatives. The Consultant Pharmacist will also participate in audits, prepare reports, and help facilities implement best practices that enhance patient safety and therapeutic outcomes.
Qualifications
- Active Pharmacist license in Illinois, with eligibility or licensure in one or more of the following states preferred: Indiana, Iowa, Missouri.
- Clinical pharmacy expertise in long-term care or similar settings, including experience with medication regimen review and drug utilization evaluation.
- Strong knowledge of state and federal pharmacy regulations, long-term care guidelines, and best practices in medication safety and compliance.
- Proficiency in documentation, report writing, and use of pharmacy or electronic health record systems to track interventions and outcomes.
- Effective communication and interpersonal skills to collaborate with interdisciplinary teams, educate staff, and engage with prescribers.
- Ability to work independently, manage multiple facilities or accounts, and balance on-site visits with remote work responsibilities.
- Doctor of Pharmacy (PharmD) or equivalent degree required; prior consultant pharmacist experience in long-term care is highly beneficial.
- Valid driver’s license and ability to travel to client facilities within the region as needed.
