The Medication History Technician obtains and documents an accurate, comprehensive medication history for patients. This position reports to the Manager of Clinical Pharmacy Services and provides a vital service to our admitted patients and providers.
- Interviews patient to obtain complete medication history, update allergy information and complete patient demographics (when needed).
- Communicates effectively outside and within the organization. Communicates with outside sources to determine all current and active medications prescribed to the patient.
- Serves as an important link in the communication chain ensuring information is transmitted between nursing personnel, providers, pharmacists, other hospital personnel, patients and family members.
- Evaluates medication, dose, route, frequency, duration of therapy and adherence.
- Accurately documents current medication history for physician reconciliation and allergy information.
- Participates in hospital and/or departmental meetings relevant to practice area.
REQUIREMENTS: Formal working knowledge; equivalent to an Associate's degree (2 years college); requires knowledge of a specialized field. 3-5 years of experience.
PTCB Certified. Maryland Board of Pharmacy Technician Registration.
