\Job Summary: We are seeking an experienced Medical Policy Pharmacist to join our team. As a key member of our pharmacy staff, you will be responsible for ensuring the safe and effective procurement, storage, distribution, and use of medications and biologicals. You will work closely with our pharmacy manager and other healthcare professionals to provide high-quality patient care.
\Responsibilities:

- Maintain accurate and up-to-date patient records, including current and past medical history, clinical data, and medication lists. \
- Develop and implement policies and procedures for medication management, including ordering, evaluation, and discussion with healthcare team members. \
- Ensure compliance with federal and state regulatory agencies and policy requirements. \
- Provide training and education to facility staff on safe and effective medication practices. \
- Collaborate with physicians, nurses, and other healthcare professionals to develop individualized care plans. \
- Monitor and evaluate patient outcomes, making recommendations for therapy changes as needed. \
- Inform the Pharmacy Manager of any medication-related errors or concerns. \
- Assist in maintaining accurate inventory levels and ordering medications as necessary. \
- Perform other duties as assigned by the Pharmacy Manager. \

Requirements: A Bachelor's Degree in Pharmacy is required, along with licensure to practice as a pharmacist in the applicable state. Experience working in long-term care settings and knowledge of I/DD populations is highly desirable.
