Responsibilities
- Engage with individuals experiencing behavioral health challenges or housing instability through outreach efforts in community settings.
- Provide peer support, mentorship, and motivational interviewing to motivate clients toward recovery goals.
- Conduct crisis intervention and crisis management when urgent needs arise, ensuring safety and stabilization.
- Collaborate with mental health professionals, addiction counselors, social workers, and housing providers to coordinate comprehensive care plans.
- Maintain accurate documentation of client interactions using electronic health record (EHR) systems while respecting confidentiality protocols.
- Assist clients in navigating behavioral health services, housing options, and community resources to promote stability and independence.
- Advocate for clients’ needs by fostering trustful relationships and empowering them to achieve their recovery objectives.
Qualifications
- Lived experience in recovery from substance use disorder or mental health challenges is highly valued.
- Prior experience in crisis intervention, crisis management, or behavioral health support is preferred.
- Knowledge of addiction counseling, mental health counseling, social work practices, and motivational interviewing techniques.
- Familiarity with EHR systems for documentation and case management purposes.
- Strong mentoring skills with the ability to motivate and inspire others toward positive change.
- Excellent communication skills to build rapport with diverse populations and collaborate effectively with multidisciplinary teams.
