Peterson Hospice is a not-for-profit rural community hospice located in Kerrville, Texas. We provide service to aptients in the following counties: Kerr, Kimble, Kendall, Gillespie, Bandera, Real, Medina, and Edwards County that are within a 50-mile radius from our office location at 250 Cully Drive, Kerrville, Texas. We serve patients in their private residence, nursing homes, and assisted living facilities. We offer mileage reimbursement at the current IRS rate. The RN Case Manager is reimbursed at an hourly rate. We utilize the HomeCareHomeBase electronic medical record system. The Hospice Admissions RN is a professional nurse who coordinates and directs the hospice patient's care in their home environment (private residence, skilled nursing facility, assisted living, hospital, etc.) based on the individual needs. The RN Case Manager is responsible for independent management of the Hospice patient population requiring use of advanced assessment, teaching and decision-making skills. The RN Case Manager cares for a caseload of hospice patients by evaluating the patient for appropriateness of hospice care and developing the plan of care in collaboration with the provider, caregivers, and Hospice Interdisciplinary Team members. The RN Case Manager provides follow up by evaluating the effectiveness of the plan of care and monitoring the patient and caregivers response to the plan. The RN Case Manager provides patient care based on provider's orders, departmental policies and procedures, regulatory guidelines and best practice. The Rn Case Manager provides care in collaboration with the provider, patient caregivers, an Hospice Interdisciplinary team members to meet the goals of care with the consideration of the patient's limitations and personal preferences in accordance with hospice philosophy. The RN Case Manager documents int he patient's medical record in a thorough and timely manner according to agency requirements. The RN Case Manager attends and actively participates in team meetings. RN Case managers routinely work, Monday through Friday 8am to 5pm, with on-call coverage on a rotating basis.
Required Education:
Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
Required Licensure/Certification/Registration:
Current Texas State license as a Registered Nurse, CPR certified, and current Texas Drivers License and auto insurance. Must have reliable transportation.
Required Work Experience:
Two years experience as an Registered Nurse with med/surg experience.
Required Technical Skills:
Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
