What are the responsibilities and job description for the Substance Use Disorder Nurse (LPN/RN) position at Personalized Nursing Light House INC?
Personalized Nursing LIGHT House is seeking qualified Full-Time, Part-Time, and Contingent LPNs or RNs to support our Men’s Residential & Withdrawal Management Program in Madison Heights.
PNLH Madison Heights provides a welcoming and structured environment for adult men beginning their recovery journey. This 30-bed program offers both residential treatment and withdrawal management services in a setting designed to promote healing, safety, structure, and hope.
Residents receive 24/7 support from a multidisciplinary team that includes nursing staff, counselors, case managers, and peer recovery coaches. Services are designed to help individuals stabilize, build coping skills, and prepare for the next step in recovery.
Preference will be given to candidates with experience in substance use disorder treatment, withdrawal management, residential treatment, behavioral healthcare, and trauma-informed care.

Available Shifts
Monday: 7:00 AM to 7:30 PM
Wednesday through Friday: 7:00 PM to 7:30 AM
Contingent: pick up open shifts as available
Responsibilities
Monitor the medical status of residents, including vital signs, medication monitoring, periodic observations, and withdrawal symptoms.
Conduct health assessments for each client admitted to the program and assist with development of the nursing care plan.
Observe and monitor clients’ physical, emotional, and behavioral status using the CIWA assessment form and other clinical protocols as appropriate.
Monitor and support clients in the self-administration of medications for symptom relief and maintain accurate medication records.
Document behaviors, physical symptoms, observations, and critical incidents in accordance with agency and clinical protocols.
Engage clients in supportive conversations related to treatment readiness, recovery, hope, and strategies for maintaining sobriety.
Develop rapport and maintain professional therapeutic relationships with clients.
Facilitate a client-centered intake process, including coordinating admission, providing program orientation, and gathering required information and paperwork.
Collaborate with counselors, peer recovery coaches, case managers, and other team members to support continuity of care.
Follow all health and safety policies and assist in maintaining a clean, sanitary, and safe environment.
Assist with household tasks as needed, including meal preparation and cleanup, laundry, and light housekeeping.
Perform other related duties as assigned by management.
Required Qualifications
Current LPN license required. RN preferred.
Strong interpersonal and communication skills.
Ability to maintain healthy and appropriate professional boundaries with clients.
Basic computer skills required.
Willingness to learn internal systems, including Microsoft Office 365 and FileMaker Pro.
Ability to pass a drug screen.
Preferred Qualifications
Experience in substance use disorder treatment, withdrawal management, detox, residential treatment, or behavioral healthcare.
Experience with CIWA assessments and withdrawal monitoring.
Training or experience in trauma-informed care.
Basic Life Support (BLS), American Heart Association certification, or CPR/AED certification.
