Registered Nurse - Transitional Care Coordinator

We are seeking a compassionate and detail-oriented Registered Nurse (RN) – Transitional Care Coordinator to join our team in Salem, CT. In this role, you will be responsible for coordinating seamless transitions of patients from hospital to home or other care settings, ensuring optimal recovery and reduced readmissions.

Key Responsibilities Include

• Assessing patient needs and developing individualized care plans
• Collaborating with healthcare providers, patients, and families to facilitate effective communication
• Providing patient education on discharge instructions, medication management, and follow-up care
• Monitoring patient progress post-discharge and addressing any issues promptly
• Documenting all care activities accurately and maintaining compliance with regulatory standards

Ideal candidates will possess strong communication, organizational, and critical thinking skills, with a focus on patient-centered care. A valid CT RN license is required.
