Key Responsibilities Include
- Assessing patient needs, preferences, and barriers to care, considering cultural, emotional, and spiritual factors.
- Developing, implementing, and documenting individualized care plans in collaboration with patients, families, and the interprofessional team.
- Promoting respect, empathy, and cultural sensitivity during care delivery, ensuring patient-centered approaches.
- Monitoring patient condition changes, updating care plans, and communicating effectively with patients and team members.
- Ensuring evidence-based practice and quality outcomes while maintaining safety in all activities.
- Valid RN license and a Nursing Diploma/Associate's degree.
- Minimum of 2 years of experience; bachelor’s degree preferred with 2-5 years of experience.
- Valid driver’s license; willingness to cover all regions during on-call.
- Registration with the Family Care Safety Registry required.
