Join a PACE program dedicated to providing outpatient and community-based care for a medically complex geriatric population. The role involves a collaborative interdisciplinary care model focused on preventive care, chronic disease management, and enhancing quality of life for elderly participants.
- Schedule: 5x8's Monday-Friday, 8a-5p, with 1-2 calls per week.
- Patient volume: 8-12 patients daily.
- Support staff includes MAs and additional support staff.
- EMR: Epic.
- Licenses/Certifications: Active CA License, BLS, DEA.
- Board Certification required; open to physicians out of residency.
- Credentialing: 2-3 weeks post-acceptance.
- Start date: As soon as possible; duration: 3 months with potential extension.
- Housing/travel: All-inclusive; pay structure: Bid.
- Notes: First week orientation at 5x8's across multiple sites; includes 1 hour daily for chart completion.
