Role: Nurse Practitioner, Advanced Practice Provider

- Provide high-quality, equitable primary care to adult Medicare patients, including routine and acute visits
- Conduct annual wellness visits, health risk assessments, and care coordination with other providers and specialists
- Build lasting patient relationships to promote trust and understanding of care plans
- Participate in population health initiatives, outreach, education, and home visits as needed
- Collaborate within a care team including physicians, nurses, medical assistants, and support staff
- Report to the Center Medical Director and work closely with operational leadership
- Master's or Doctorate in Nursing or NP certification in Family, Adult-Gerontology, or Gerontological specialties
- Active, non-probationary NP license and DEA registration
- US work authorization
- Preferred: experience in primary care, understanding of HCC, ICD-10 coding, and population health
- Team-oriented, innovative, and bilingual (preferred)
- Full-time position, approximately 40 hours/week
