Role: Nurse Practitioner in Senior Community Care

Responsibilities

  • Provide primary and preventive care to seniors enrolled in the PACE program
  • Perform assessments, diagnose, and manage acute and chronic conditions
  • Prescribe medications and develop individualized care plans
  • Collaborate with a multidisciplinary team including physicians, nurses, social workers, therapists, and caregivers
