Job Overview:

As a Clinical Fellowship Speech-Language Pathologist (CF-SLP) with Personal Touch Home Care, you will provide speech, language, cognitive-communication, and swallowing services to pediatric and adult patients, including individuals with special needs, within their home environments. This position allows you to complete your Clinical Fellowship under the guidance and supervision of a licensed Speech-Language Pathologist, offering mentorship, clinical feedback, and professional development.

In this role, you will help patients improve communication abilities, functional independence, and overall quality of life through individualized therapy delivered in the comfort and familiarity of their homes. You will work with a diverse population, including children with developmental and communication disorders as well as adults with neurological, cognitive, or medical conditions affecting speech and swallowing.

The Clinical Fellowship experience is designed to support your transition from graduate training to independent clinical practice while developing strong skills in assessment, treatment planning, patient education, and interdisciplinary collaboration.

Key Responsibilities

  • Conduct speech, language, cognitive-communication, and swallowing evaluations for pediatric and adult patients, including individuals with developmental disabilities, neurological conditions, and other special needs, under the guidance of a supervising Speech-Language Pathologist.
  • Develop and implement individualized treatment plans based on patient-specific goals, functional abilities, and home environments.
  • Provide speech and language therapy addressing articulation, language development, fluency, voice, social communication, cognition, and swallowing disorders.
  • Deliver therapy services in a home care setting, adapting interventions to support daily communication and functional independence.
  • Work with pediatric patients with developmental delays, autism spectrum disorders, and other communication challenges, as well as adults with conditions such as stroke, traumatic brain injury, dementia, or other neurological impairments.
  • Collaborate with caregivers, family members, and interdisciplinary healthcare professionals to provide coordinated and holistic care.
  • Educate patients and families on communication strategies, therapeutic activities, and home programs to reinforce progress outside of therapy sessions.
  • Monitor and document patient progress, adjusting treatment plans as needed in consultation with the supervising clinician.
  • Maintain accurate clinical documentation in compliance with agency policies and regulatory standards.
  • Participate in regular supervision and mentorship meetings to support clinical skill development and successful completion of Clinical Fellowship requirements.
  • Stay current with evidence-based practices and clinical advancements in speech-language pathology.

Qualifications:

  • Master’s degree in Speech-Language Pathology from an accredited college or university.
  • Eligible for Clinical Fellowship (CF) licensure as a Speech-Language Pathologist in the state of practice.
  • Strong verbal, written, interpersonal, and organizational communication skills.
  • Ability to work independently in a home care environment while maintaining collaboration with supervising clinicians.
  • Interest or experience working with special needs populations across the lifespan, including pediatric and adult patients.
  • Experience in home health, rehabilitation, pediatrics, or developmental disabilities is preferred but not required.
  • New graduates seeking to complete their Clinical Fellowship in a supportive home care setting are encouraged to apply.
