What are the responsibilities and job description for the Direct Support Worker position at Personal Care Solutions?
OVERVIEW: This position entails supporting a young man through providing personal care supports and learning independent living skills in his private residence. The Direct Support Worker (DSW) will provide direct assistance in multiple activities of daily living; supporting personal hygiene, basic daily and instrumental living functions and ensuring general health and safety. Support may also include supporting the individual to advocate for himself, learn new skills, practice independence and participate in community, educational or enrichment activities.
OPEN HOURS:
6am – 9am, Monday - Friday
3pm – 11pm, Sunday - Friday
MINIMUM QUALIFICATIONS:
- At least 3 years of experience working with individuals with mobility and fine motor skill impairments in residential settings
- Valid driver’s license, clean driving history, reliable and timely transportation to and from shifts
- Must be able to work in a variety of settings, weather conditions and locations
- Must be able to perform personal care duties including toileting, dressing and grooming
- Ability to communicate clearly, both in-person and in writing on at least a weekly basis
- Possess strong time-management and organizational skills
- Work a consistent and sometimes flexible schedule that may include weekends, on-call, evenings or holidays
- Ability to provide physical assistance, supporting up to 50 pounds such as lifting or supporting in an emergency which may include first aid or evacuation techniques
- Work independently on shift, strong judgment skills and possess a strong sense of initiative
- Final candidates will be asked to participate in the pre-employment background checking processes, including but not limited to fingerprinting and driving records.
JOB RESPONSIBILITIES:
- Must be able to perform instrumental activities of daily living including supporting the individual with activities such as housework, laundry, meal planning, etc.
- Provide hands-on personal care which will include dressing, personal hygiene and other activities of daily living
- Assist with meal planning and preparation on a regular basis
- Support and encourage the individual to problem-solve, where appropriate, and identify how to function with independence and greater self-reliance
- Support and attend medical or community-based appointments as needed
- Provide counseling, mentoring and training in the areas identified by the individual, his family and his support team
- Provide assessment, feedback and follow-up through clear and concise communication before, during and after shifts with appropriate team members
- Encourage self-awareness and self-direction skills during shifts & activities
- Operate accessible wheelchair van, provide transportation to and from activities in the individual's vehicle
- Provide one-to-one assistance as needed to the individual within the home and community
- Ensure the individual's safety while in the community
- Support the individual to enhance his social, communication and problem-solving skills
