Job Title: Intake Coordinator
Position Summary
The Hospice Intake Coordinator is responsible for managing the intake and admissions process for patients referred to hospice services. This role serves as a central point of contact for referral sources, patients, families, and the interdisciplinary team, ensuring timely, accurate, and compassionate coordination of admissions, scheduling, and staffing while maintaining compliance with hospice regulations and organizational policies.
Essential Duties & Responsibilities
- Receive and process hospice referrals from hospitals, physicians, facilities, patients, and families
- Verify patient eligibility for hospice services based on clinical information and physician orders
- Coordinate and schedule hospice admissions, visits, and start-of-care services in collaboration with clinical leadership
- Assign and coordinate appropriate clinical staff (nurses, aides, social workers, chaplains, and other team members) to meet patient needs and ensure coverage
- Communicate scheduling changes, urgent admissions, and staffing needs to the interdisciplinary team in a timely manner
- Collect, review, and accurately enter patient demographic, clinical, and insurance information into the electronic medical record (EMR)
- Verify insurance coverage, obtain authorizations when required, and communicate benefits to patients and families
- Serve as a liaison between referral sources, patients, families, and hospice team members
- Ensure all required documentation is completed prior to or at admission
- Track, follow up, and resolve pending referrals, scheduling conflicts, and incomplete documentation
- Maintain compliance with Medicare, Medicaid, and hospice regulatory requirements related to intake and admissions
- Provide compassionate, clear communication to patients and families regarding hospice services and care coordination
- Participate in quality improvement activities related to intake, scheduling, and staffing workflows
- Maintain confidentiality and adhere to HIPAA regulations at all times
Qualifications
- High school diploma or equivalent required; associate or bachelor’s degree in healthcare or related field preferred
- 1–2 years of experience in hospice, home health, healthcare intake, scheduling, or staffing coordination preferred
- Knowledge of hospice philosophy, Medicare hospice guidelines, and insurance verification processes preferred
- Strong organizational, scheduling, and time-management skills with the ability to manage multiple priorities
- Excellent verbal and written communication skills
- Proficiency with electronic medical records and scheduling systems
- Ability to work independently and collaboratively within an interdisciplinary team
- Compassionate, professional demeanor with sensitivity to patients and families during end-of-life care
- Strong attention to detail and problem-solving skills in a fast-paced environment
