Registered Nurse / RN
Position Summary
- Responsible for assessment and coordination of individual’s health care needs as assigned.
- Provide expert consultation, training, and guidance to staff regarding medication administration and health issues for individuals supported by agency
- Maintains the vision, mission, and values of Person Directed Supports Inc. and provides the highest quality of care consistent with PDSI values and standards.
Job Responsibilities
- Establish supportive and motivational relationships with the individuals we support, ensuring an atmosphere of safety, security, and responsible medical care.
- Interact with Physicians to ensure the optimal level of health care for the people we support.
- Know current level of medical functioning of individuals being supported and appropriate needed interventions.
- Management of HRST system for Individuals.
- Assist individuals being supported and staff in understanding and management health, meet regularly with assigned person managing symptoms of their health.
- Assist in the completion of medical protocols service plans with the individuals we support and team members.
- Perform ongoing assessments for medication side effects and drug interactions.
- Process medication changes; make follow-up calls, as needed.
- Facilitate and coach the individuals being supported in using natural supports and mainstream community resources to supports goals around living in the community.
- Keep current on developments in medications and health care related to medical health and wellness and train each staff.
- Ensure staff responsible for medication monitoring are appropriately trained, including proper reporting procedures for problems with self-administration and proper responses to medical emergencies
- Will serve as medical on-call officer
- Have ability to intervene appropriately in all medical issues when necessary.
- Be available for emergencies on short notice, if necessary.
- Complete required training, both upon hire and annually
- Excellent interpersonal skills with people from differing backgrounds and cultures.
- Ability to be flexible in meeting scheduling needs.
- Ability to maintain confidentiality of the people we support; information and appropriate boundaries.
- Maintain the vision, mission, and values of Person Directed Supports Inc
- Remain up to date regarding various the programs of Person Directed Supports Inc as well as remain up to date with and perform all functions in accordance with PDSI policies and procedures.
Requirements
MINIMUM QUALIFICATIONS:
- Licensed, Registered Nurse in the state of Pennsylvania
- 2 years’ experience working with people in the Human Services field.
OTHER REQUIREMENTS:
- Ability to maintain a clean driving record, a current drivers’ license, and auto insurance coverage.
- Ability to perform on-call duties
