Are you passionate about providing compassionate, person-centered healthcare to individuals with Intellectual and Developmental Disabilities (IDD)? We are seeking a dedicated Nurse Practitioner to join our interdisciplinary team and help improve the health, independence, and quality of life of the individuals we support.
This position provides comprehensive primary care services while collaborating with nurses, behavioral specialists, direct support professionals, families, and community healthcare providers to ensure exceptional care for adults living in community homes.
Essential Responsibilities
- Provide comprehensive primary care services, including assessments, diagnosis, treatment, and follow-up care.
- Conduct annual physical examinations and routine wellness visits.
- Evaluate and manage acute and chronic medical conditions.
- Prescribe medications and monitor treatment effectiveness.
- Develop and maintain individualized treatment plans.
- Collaborate with interdisciplinary teams to coordinate holistic care.
- Provide education and guidance to individuals, families, guardians, and support staff regarding medical conditions, medications, and preventive healthcare.
- Participate in care planning meetings and interdisciplinary team meetings.
- Ensure documentation is accurate, timely, and compliant with organizational policies and state regulations.
- Support preventive healthcare initiatives, including immunizations, screenings, and health promotion activities.
- Coordinate specialty referrals and communicate with outside providers as needed.
- Participate in quality improvement initiatives and assist with reducing avoidable hospitalizations and emergency room visits.
- Remain current on best practices in caring for individuals with intellectual and developmental disabilities.
Qualifications Required
- Master's or Doctorate degree from an accredited Nurse Practitioner program.
- Current Nurse Practitioner license in the Commonwealth of Pennsylvania.
- National Board Certification as a Nurse Practitioner.
- Current DEA registration (or ability to obtain).
- Valid driver's license with an acceptable driving record.
- Ability to travel to multiple community homes throughout the service area.
Preferred
- Experience working with individuals with Intellectual and Developmental Disabilities (IDD), Autism Spectrum Disorder (ASD), or behavioral health populations.
- Experience in family practice, internal medicine, community health, or geriatrics.
- Knowledge of Pennsylvania Office of Developmental Programs (ODP) regulations and person-centered practices.
- Experience working within interdisciplinary healthcare teams.
