Position Summary

The Registered Nurse (RN) – Wound Care, Case Management, Utilization Review, and Outpatient Infusion Services provides specialized clinical care and care coordination across the continuum of care for patients in the Critical Access Hospital, Swing Bed Program, Emergency Department, and Outpatient Services. This position serves as the clinical expert in wound prevention and management, coordinates referral process, assists with discharge planning and transitions of care, performs utilization review activities, and oversees outpatient infusion and treatment services while promoting quality outcomes, patient safety, regulatory compliance, and exceptional patient-centered care.

Wound Care Services

  • Perform comprehensive skin and wound assessments.
  • Develop and implement evidence-based wound treatment plans.
  • Measure, stage, photograph, and document wounds.
  • Perform wound care procedures and monitor healing.
  • Educate patients, families, and staff.
  • Lead pressure injury prevention initiatives.
  • Track wound outcomes and serve as the wound care resource.

Case Management

  • Coordinate patient care from admission through discharge.
  • Coordinate and implement discharge planning needs/processes.
  • Coordinate referrals and transitions of care.
  • Participate in interdisciplinary care conferences.
  • Reduce avoidable readmissions through effective care coordination.

Utilization Review

  • Perform medical necessity reviews using approved criteria.
  • Review admissions, observation status, continued stays, and Swing Bed eligibility.
  • Assist providers with documentation supporting medical necessity.
  • Participate in Utilization Review Committee activities.
  • Support denial prevention and regulatory compliance.

Outpatient Infusion & Treatment Services

  • Coordinate outpatient infusion schedules.
  • Review provider orders and collaborate with pharmacy and business office.
  • Administer IV medications, antibiotics, hydration, injections, blood products (if applicable), and other prescribed therapies.
  • Manage peripheral IVs, PICCs, midlines, implanted ports, and vascular access devices within competency.
  • Monitor for infusion reactions and initiate emergency interventions as needed.
  • Provide patient education and coordinate laboratory testing.
  • Maintain accurate documentation and support expansion of outpatient infusion services.

Quality Improvement & Leadership

  • Participate in QAPI and infection prevention initiatives.
  • Prepare for regulatory surveys.
  • Serve as a clinical resource and collaborate with interdisciplinary teams.
  • Perform additional duties assigned by the Chief Nursing Officer.

Minimum Qualifications

Graduate of an accredited School of Nursing; current Nevada RN license; BLS required; ACLS preferred. BSN preferred.

Preferred Experience

Clinical experience in wound care, case management, utilization review, outpatient infusion, Critical Access Hospital operations, Swing Bed services, and quality improvement.
