Key Responsibilities
- Conduct community-based outreach and assessments to determine potential members’ eligibility and safety for program enrollment.
- Collaborate with intake, outreach, and clinical teams to coordinate appropriate care and services for new members.
- Educate potential members and their families about program benefits, enrollment processes, and available services.
- Maintain timely and accurate documentation of all assessments and communications in accordance with program standards.
- Serve as a liaison between members, families, physicians, and referral sources to ensure a seamless intake experience.
- Active New York State Registered Nurse (RN) license.
- Associate’s Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 1 year of nursing experience in an acute care setting.
- UAS-NY (Uniform Assessment System – New York) experience required.
- Home care or care management experience preferred.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook).
- Excellent verbal, written, and interpersonal communication skills.
- Bilingual in Chinese or Spanish required.
