Overview
Position Summary
We are seeking a compassionate, dependable, and qualified Registered Nurse (RN) or Licensed Practical Nurse (LPN)to provide nursing services and clinical oversight to individuals residing in a Community Residential Services (CRS) facility.
The RN/LPN will work closely with residents, direct support professionals, supervisors, healthcare providers, pharmacies, and families/guardians to ensure residents receive safe, appropriate, and person-centered care.
Key Responsibilities
- Provide nursing care and health monitoring to residents.
- Administer and monitor medications in accordance with physician orders and facility policies.
- Ensure accurate and timely medication administration and documentation.
- Review Medication Administration Records (MARs) and address discrepancies or errors.
- Monitor residents for changes in physical, mental, or behavioral health.
- Communicate significant health changes to appropriate healthcare providers, supervisors, and guardians as required.
- Coordinate and follow up on medical, dental, psychiatric, and other healthcare appointments.
- Maintain accurate and timely nursing documentation and health records.
- Develop, review, and update nursing-related care plans and health protocols as needed.
- Provide direction, training, and education to direct support staff regarding residents' healthcare needs.
- Ensure staff understand and follow medication, health, infection-control, and emergency procedures.
- Respond appropriately to medical emergencies and coordinate emergency services when necessary.
- Communicate with pharmacies, physicians, clinics, hospitals, and other healthcare professionals.
- Monitor chronic health conditions and follow prescribed treatment plans.
- Assist with maintaining compliance with applicable state regulations and licensing requirements.
- Protect resident privacy, dignity, rights, and confidentiality at all times.
- Participate in team meetings and collaborate with the care team to promote positive outcomes for residents.
Qualifications
- Current and valid RN or LPN license in the State of Minnesota.
- Current CPR/First Aid certification or willingness to obtain it.
- Experience working with individuals with disabilities, mental health needs, behavioral health needs, or other supportive-care needs is preferred.
- Strong medication administration and documentation skills.
- Ability to recognize and appropriately respond to changes in a resident's health status.
- Strong communication, organization, and time-management skills.
- Ability to work independently while also collaborating effectively with a multidisciplinary team.
- Must demonstrate professionalism, reliability, compassion, and respect for the individuals receiving services.
Preferred Experience
- Community Residential Services (CRS) experience.
- Group home, assisted living, behavioral health, or developmental disability services experience.
- Experience with medication management and MAR documentation.
- Experience training and supervising direct support professionals.
- Experience working with person-centered service plans and individualized care plans.

Pay: $36.81 - $44.32 per hour
