Registered Nurse or Licensed Practical Nurse Home Health | Position Details
The RN or LPN Case Manager provides skilled nursing care, patient education, and care coordination in collaboration with the healthcare team to support optimal patient outcomes.
- Assess patient and family needs and develop individualized care plans
- Deliver skilled nursing care and health teaching
- Monitor patient condition and report significant changes
- Maintain accurate documentation of visits and care provided
- Coordinate care with physicians and interdisciplinary team
- Supervise Home Health Aides and support team collaboration
- Participate in case coordination, quality improvement, and staff support
- Assist with staff training, onboarding, and performance evaluation
- Perform additional duties and leadership responsibilities as assigned
What You Offer
- Graduate of an accredited nursing program
- Current RN or LPN license in Minnesota
- Valid driver’s license and auto insurance; ability to travel in all weather conditions
- Strong clinical, communication, and organizational skills
- Ability to work independently and maintain professionalism and confidentiality
- Communicate effectively in English, verbally and in writing.
- Pass a background study through the Minnesota Department of Human Services.
- Patient-focused care and strong clinical judgment
Work Schedule
- Day Shift - 8am-4:30pm
- Casual or Roster Status
Compensation
- Registered Nurse: $41.82-$57.64 DOE
- Licensed Practical Nurse: $26.99-$37.21 DOE
