Job Summary
Performance Home Health is seeking an experienced and motivated full-time Field (RN) Registered Nurse to join our home health team in the Dallas-Fort Worth Metroplex. The RN will provide skilled nursing care to patients in their homes while ensuring high-quality clinical outcomes, accurate documentation, and adherence to productivity standards. This role requires strong clinical judgment, independence in the field, and consistent collaboration with clinical leadership and the interdisciplinary care team.
Responsibilities
- Complete Start of Care (SOC), recertifications, and discharge assessments
- Develop, implement, and update individualized plans of care in collaboration with physicians and care teams
- Provide wound care and wound vac management
- Provide PICC and port care
- Provide IV therapy and injections
- Provide Catheter care
- Provide Patient and caregiver education
- Monitor patient condition and communicate changes to physicians and clinical leadership
- Ensure timely, accurate EMR documentation per agency standards
- Coordinate care with interdisciplinary team members (therapists, case managers, staffing)
- Educate patients and families on disease management, medications, and self-care
- Adhere to HIPAA, infection control, and safety standards
- Participate in case conferences, in-services, and quality improvement initiatives
- Productivity & Visit Expectations
- Meet weekly productivity (30 points) and visit expectations
- Maintain timely completion of all documentation
- Communicate proactively regarding scheduling challenges or missed visits
- Support coverage needs to ensure continuity of care
Required Experience
- Active RN license in the state of Texas
- Minimum of 2 years of RN experience
- Must have at least 1 year of home health experience
- Wound care and/or wound vac certification
- IV therapy or infusion experience
- PICC line and implanted port care experience
- IV therapy administration and injectable medication administration experience
- Strong clinical assessment and critical thinking skills
- Able to work independently in the field
- Proficient with EMR systems
- Understanding of home health documentation and regulations
- Valid driver’s license and reliable transportation
- Strong organizational and time management skills
- Knowledgeable of HIPAA compliance
Preferred Experience
- Experience with KanTime EMR
- LVAD experience
- Bilingual (preferred but not required)
- Work Environment
- Remote - Field-based role (travel required with DFW area)
- Independent home visits across assigned territory
Salary Compensation
- Pay: $100,000
