Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure comfort and dignity
- Monitor and observe patient conditions, including vital signs and overall health status, reporting changes promptly to healthcare professionals
- Support medication management by administering prescribed medications following medical instructions and documentation protocols
- Help with mobility and transfers using patient transfer methods and equipment like Hoyer lifts or gait belts to promote safety
- Maintain accurate medical records and documentation using Electronic Health Record (EHR) systems while adhering to HIPAA regulations to protect patient confidentiality
- Provide companionship and emotional support, engaging patients in conversation or activities to enhance mental well-being
- Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe living environment
Requirements
- Proven experience working with individuals with disabilities, dementia care, Alzheimer's care, or geriatrics in home or long-term care settings
- Knowledge of medical terminology, home healthcare regulations, and patient care standards
- Certification in First Aid and Basic Life Support (BLS), including CPR training
- Ability to perform heavy lifting safely and use assistive devices like Hoyer lifts or patient transfer techniques effectively
- Strong communication skills for client interaction and coordination with healthcare teams
- Experience working with EMR/EHR systems for accurate medical documentation
- Valid driver’s license and reliable transportation preferred for client visits within the community
- Compassionate attitude with a commitment to maintaining patient confidentiality under HIPAA guidelines
