Personal Care Assistant

Salary: $20Location: Client Homes (Madison)Hours: Varies Based on Client Needs and Care PlanJob Summary:The Personal Care Assistant (PCA), Homemaker, and Respite Staff is responsible for providing essential in-home support services to Perfect Care Inc. clients. This includes assistance with personal care needs, activities of daily living (ADLs), home maintenance, and safety monitoring. All duties are performed in accordance with each clients individualized Plan of Care and the PCAs certified Scope of Practice.Key Responsibilities:Quality of Care:Assist clients with personal care activities including:Bathing, oral care, denture care, skin care, and groomingBathroom support, dressing, undressing, transferring, and ambulationSupport home and environmental tasks including:Meal planning and preparationFeeding, changing linens, making beds, and light housekeepingLaundry of clothes and beddingEnsure client safety, including proper use of equipment (e.g., side rails, wheelchairs, footstools)Follow the individualized Plan of Care as outlined on the Aide Activity SheetMonitor and observe clients conditions and report any concerns, changes, or incidents to the clinician immediatelyDocumentation:Complete and maintain all required records and reports in compliance with agency policiesAccurately document all services rendered on the day they are providedEnsure documentation is clear, legible, and submitted in a timely mannerCommunication:Maintain open and professional communication with the client, their family, and the supervising teamCollaborate with members of the care team and participate in case conferences when requestedNotify supervisors promptly of any concerns or client care issuesQualifications:High school diploma or equivalent preferredCertification as a Personal Care Assistant (PCA) or Home Health Aide (HHA) in good standingExperience in homecare, elder care, or personal support services preferredAbility to read, write, and follow verbal and written instructionsStrong interpersonal and communication skillsReliable transportation and willingness to travel to client homes
