We are currently seeking a dedicated and passionate Case Manager with experience in senior living to join our team.
Case Manager Responsibilities
- Orient and assist each resident as they adjust to life in the community.
- Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to the community, including addressing their social, emotional, and psychological needs.
- Assist each resident in maintaining family and community connections while encouraging the development of new relationships.
- Complete quality management reviews to ensure the department meets state regulations and Peregrine Senior Living policies, and follow up as needed.
- Collaborate with the care department and other team members to promote and protect resident rights and support each resident's psychological well-being.
- Bachelor's degree with major coursework in Human Services, and at least one year of full-time work experience in a related field.
- Case management experience required.
- Memory Care or Assisted Living experience required.
- Compassion for and a genuine desire to work with older adults.
