Patient Clinic Representative - Referral
FLSA Classification: Non-exempt
Reports to: Clinic Support Supervisor
Job Summary/Objective
The PCR Referral position facilitates patient care by managing the workflow for all incoming referrals, coordinating appointments with patients and external providers, ensuring timely communication and follow-up, and supporting referring providers with accurate and prompt updates. Performs all defined services and other related duties in accordance with the mission of Peoples Community Health Clinic (PCHC).

This position requires occasional standing -(1%-33% of shift); frequent sitting (34% - 66% of shift; occasional walking -(1%-33% of shift); occasional lifting and carrying up to 10 lbs. (1% - 33% of shift); frequent pushing/pulling up to 10 lbs.(34%-66% of shift) and occasionally 11lbs. - 100lbs (1% - 33% of shift); frequent use of hands/arms for repetitive movement and continuous finger dexterity; occasional bending/stooping, twisting, squatting/crouching, and reaching at, below and above shoulder level (1% - 33% of shift).
Position Type/Expected Hours Of Work
This is a full-time position. Typically work hours and days are between Monday and Friday,8:00 a.m. to 5:00 p.m. and some flexibility in hours is allowed. Responsibilities occasionally may require irregular hours as deemed necessary. This may include an adjusted work schedule, long days, and early morning or late evening hours in order to meet client scheduling demands. The employee must work a minimum of 40.0 hours each week to maintain full-time status.
Travel
Occasional driving might be expected for this position.

- Receive, review, and process all incoming referrals according to department protocols in a timely and accurate manner.
- Referrals in the work queue must be addressed within 5-7 business days.
- Coordinates appointments for PCHC patients between their provider and outside referral sources to include, but not limited to:
- Verification of patient demographic information and health insurance coverage. Use this information to determine the best service for patient needs, ensuring patients attend appointments as scheduled.
- Maintains timely correspondence utilizing phone, email, and electronic messaging systems, between PCHC providers, insurance providers, patients, and referral sources at the frequency necessary to complete patient referral.
- All emails and voicemails addressed on a daily basis
- In-basket/EPIC messages will be responded to or addressed within 5 business days.
- Identify and prioritize referrals marked as "urgent" and process according to established workflow standards and timelines so as not to interfere with or impact patient care.
- "Urgent" messages will be addressed within 24 hours.
- Maintain accurate and up to date documentation in the Electronic Health Record (EHR) and work queues.
- Maintains the organization and cleanliness of all workspaces and keeps only supplies necessary for the position in work area.
- Supports a service-oriented atmosphere in accordance with Peoples Community Health Clinic Mission and Philosophy.
- Works to improve work processes and clinical outcomes including health disparity and quality improvement collaboratives.
- Follows PCHC policies and procedures.
- Maintains a safe working environment and practices safe working habits.
- Assists in control of Peoples Community Health Clinic resources.
- Proficient in verbal and written communication, including active listening and reading comprehension.
- Skilled in planning, organizing, adaptability, attentiveness to detail and flexibility of assignment.
- Ability to work with and communicate with diverse groups of people effectively and respectfully.
- Basic skills required including working knowledge of MS Office Products (ex. Excel, Word, Outlook) and Windows Platform.
- Demonstrated EHR proficiency according to the functions of the position and adherence to EHR documentation standards to ensure effective and compliant use of the EHR in daily operations.
- Ability to work with a large degree of independence, managing the assigned workload without regular assistance from team members.
- Demonstrates a cooperative and courteous manner towards staff, patients, families, and visitors while carrying out job functions.

Education and Experience
- High School diploma or GED equivalent required.
- Completion of Nurse Aide I course, Medical Assistant program, or 1-year direct care in medical office preferred.
- Minimum of 1 year patient care or medical experience preferred.
- Within the first 90 days of employment, employee must be able to demonstrate competency in all Electronic Health Record (EHR) functions relevant to the role, including documentation accuracy, data entry, and system navigation. After training, competency is verified through practical demonstration and/or skills assessment.
