Job Title: Intake Coordinator
Job Summary:
- The intake coordinator is responsible for receiving all referrals, recording details of the referral, gathering background information, coordinating intake appointments, client satisfaction, and ensuring complete intake of new clients with the proper agency. To be successful as an intake coordinator, you should be highly organized and detail oriented. Top candidates will also have excellent interpersonal skills, with a strong aptitude for great customer service.
Duties/Responsibilities:
- Intake Coordinator o Referrals
- Receive incoming client referrals via phone, email, and fax
- Conduct initial referral calls, determine service needs, and enter referral into Catylyst
- Complete and follow the intake procedure
- Communicate with the appropriate agency regarding staffing, services, and start of care
- Maintain a good working rapport with referral partners, case managers, and SEP’s o Intakes
- Schedule initial service intake assessment
- Audit new admission paperwork to ensure a complete client record
- Make welcome calls to clients/family and answer any final questions o Client Communications
- Reviewing and addressing, as appropriate, any complaints and or incidents related to the intake process
- Responds promptly and courteously to all client inquiries or problems
- Customer service check in calls, monthly
- Provide client resources, refer as needed
- General
o Attend meetings as required by Supervisor
- Business Development o General Business Development Process:
- In coordination with local office or their appropriate designate, provide direction and support to the Business Development process, including proper documentation and operational efficiencies
- Maintain Business Development lists and contacts for potential referral sources, (i.e. legal, hospice services, Medicare, ancillary services, etc.) o Targeted Business Development Efforts: Facilitate and/or support strategic and/or urgent Business
Development efforts for agencies o In coordination with the Co-CEO provide tacit support for those marketing efforts synergistic with the
Business Development efforts and provide regular reports as directed.

Education:
- High school diploma or equivalent required
- Relative academic, vocational, and/or professional education
Experience:
- Minimum of one-year experience in the provision of medical or non-medical home care
- Or, One-year experience in patient intake or medical office administration
Working Conditions:
- Prolonged periods of sitting at a desk and working on a computer.
- Essential physical requirements, such as standing, kneeling, squatting, stooping, or typing.
- Physical effort/lifting, such as sedentary - up to 10 pounds
- May be required to work weekends, nights, or be on-call as a regular part of the job.
