People Places and Dreams, a peer-driven OHMAS-accredited mental health and substance abuse agency, seeks talented, highly motivated professional individuals to join our team of dynamic CDCAs, Case Managers, and Clinicians. We are looking for a Qualified Mental Health Specialist who understands and respects the importance of self-directed paths to recovery, recognizes the transformative power of hope and encouragement through sharing lived experiences, and, most importantly, can model and advocate health and wellness through the implementation of the eight dimensions of wellness and supporting clients on their journey to recovery. As a QMHS, you will operate under the general supervision of the Clinical Supervisor and are required to manage data and attend staff meetings and training to ensure credential maintenance. Must have the willingness to work with underserved populations, including persons who utilize MAT or identify as LGBTQ or have co-occurring disorders. We have dynamic shifts, including part/full-time hours, days, evenings, nights, and weekends. This position is not responsible for direct supervision. This position will report to the clinical director as their direct supervisor. The Clinical Director or Case Management Supervisor supervises the assigned position. Duties and Responsibilities:
- Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
- Handle case assignments, draft service plans, review case progress, and determine case closure
- Help clients achieve wellness and autonomy
- Facilitate multiple care aspects (case coordination, information sharing, etc)
- Help patients make informed decisions by acting as their advocate regarding their clinical status and treatment options
- Develop effective working relations and cooperate with the medical team throughout the entire case management process
- Arrange abeyances with social services, health, and governmental agencies
- Take the extra mile and interact with patients to keep track of their progress and to ensure satisfaction
- Record case information, complete accurately all necessary forms, and produce statistical reports
- Promote quality and cost-effective interventions and outcomes
- Assess and address motivational and psychosocial issues
- Adhere to professional standards outlined by protocols, rules, and regulations. Knowledge and skills to teach and engage in basic problem-solving strategies to support individuals in self-directed recovery.
- Ability to assist individuals in addressing symptoms using strategies such as positive self-talk.
- Knowledge and skills sufficient to use community resources necessary for independent living and the ability to teach those skills to other individuals with severe mental illness.
- Knowledge of how to establish and sustain self-help (mutual support) and educational groups by soliciting input from individuals on their strengths and interests.
- A valid driver's license is required as some driving and/or transportation may be required to take clients to medical appointments, job sites, social activities, and other community resources.

Educational/Training Requirements:
- A bachelor's degree or higher is preferred but not required.
Experience:3-4 years of proven working experience in case management is a plus.
Licenses/Certifications:
None required. CDCA, LSW, LPC, LPCC, LISW Strongly preferred

· 80% or more travel within Franklin County is expected

Pay: $24.00 - $25.00 per hour
