Job Overview
Our Case Managers collaborate with our internal treatment team and community agencies to directly advocate for at-risk children and families, providing critical needs assessments, prevention and intervention services, and progress monitoring for the Department of Child Services and Juvenile Probation.
Family Progress - Monitoring and Coaching (50 %)
- Coach collaboratively – maintaining a balance between teaching and serving
- Visit each youth/family on a routine and 'pop in' schedule
- Assist youth/families in obtaining their goals
- Provide education and support to youth/families
- Provide transportation to youth/families as needed
- Attend court with clients and render recommendations
- Communicate with schools and various institutions related to youth/family matters
- Respond to all crisis calls and manage those situations
- Service Plans / Goal Development (20 %)
- Create plans, including goals, for each youth/family
- Review and update plans and goals
- Develop a network of relationships with agencies in the community
- Work collaboratively with other social welfare agencies
- Refer when appropriate to community agencies based on needs
- Respond to crisis calls received via pager and manage situations
- Intake and Needs Assessment (15 %)
- Determine 'crisis' situations
- Visit each youth/family to gather necessary information to complete the assessment
- Adhere to and follow People Makers professional standards
- Reporting (15 %)
- Maintain case and agency paperwork according to People Makers standards
- Prepare written reports for the Department of Child Services / Probation office within identified timeframes

Pay: $35.00 - $50.00 per hour
