- Strong skills and experience in Discharge Planning to ensure optimal continuity of care

- Active RN license with a robust background in Nursing and medical best practices
- Exceptional communication and organizational abilities
- Familiarity with healthcare regulations and compliance standards
- Bachelor of Science in Nursing (BSN), or equivalent, is highly preferred
