Case Manager - Day Habilitation

SUMMARY: The case manager assumes primary responsibility for each member on an assigned caseload and coordinates the implementation of the program Service Plan and maintains each members record.
ESSENTIAL FUNCTIONS:
- Develop and ensure the implementation of the program Service Plan.
- Serve as liaison between the program and each members support system.
- Complete and submit all assessments as required.
- Maintain and update all service records.
- Maintain an expertise in the field through attendance at meetings, trainings, continuing education or other
- appropriate educational programs.
- Attend interdisciplinary team meetings as needed to participate in curriculum design.
QUALIFICATIONS:
- High school diploma or equivalent
- At least one-year case management experience in a program for developmentally disabled adults, or three years experience in Human Services or a related field.
- Skill in both written and oral communication.
- Ability to working both independently and as a team member.
- Ability to effectively participate in the design, implementation, and coordination of services.
- Ability to complete and disseminate all required forms and documents.
- A qualifying criminal background check, driving record, and a valid drivers license required.
