POSITION OVERVIEW
Through the Baltimore Comprehensive Overdose Response to End the Epidemic (BCORE) partnership, PEP will implement a mobile interdisciplinary model for outreach, substance abuse and intensive case management to people at high risk of opioid overdose in close coordination with EMS and hospital Emergency Departments. The BCORE Outreach Director will lead the implementation of the model and act as liaison to strategic partners to ensure ongoing success, sustainability, and alignment with City goals.
BCORE is a grant to end the opioid epidemic, including enhanced opioid crisis response and community outreach, improved standards of care for additional care in the city’s emergency rooms, a 24/7 access line for treatment and support services, and a software platform that connects individuals across a range of city sectors engaged in this work.

RESPONSIBILITIES
- Elevate PEP’s mission through role modeling, storytelling, collaborative case conferencing, and otherwise embodying the humility, spirit and innovation of street outreach services, harm reduction, nonjudgemental trauma-responsive care, and radical hospitality.
- Continuously seek new insights, options, and approaches to reach hard-to-engage clients with the philosophy that we never give up on anyone.
- Earn and maintain clients’ trust through consistency, integrity, honesty, managing expectations, unconditional positive regard, and persistent follow-up; impart this approach throughout the team through role modeling, coaching, and close consultation.
- Hire, train, and supervise outreach staff; evaluate staff performance and initiate improvement actions as required.
- Direct the day-to-day operations of the team including scheduling, canvassing high-priority locations, and meeting with program partners.
- Track and ensure time-sensitive and appropriate urgency to follow-up to referrals and excellent communication and coordination.
- As needed, directly provide services including outreach, engagement, case management, care coordination, and crisis intervention/management including on-call duty on nights and weekends and during emergencies.
- Initiate professional development and clinical consultation to improve clinical insights, creative interventions, and methods regarding mental wellness, addictions, and medical needs of the unhoused population.
- Engage in professional development and advocacy to improve case management processes and systems navigation, especially benefits, housing, legal, and employment support systems.

- Ensure departmental compliance with contract terms, laws, regulations, and funder guidance.
- Ensure departmental and/or sub-team processes for daily or weekly roster/case reviews
- Manage a data strategy and plan for the department that ensures effective utilization of all required data systems, produces data elements required for reporting, and creates data analytics needed for program evaluation, public health improvement, and system design.
§ Assess, design, implement, continuously monitor, and improve data and technology processes and systems as needed and as required by PEP funders and PEP Senior Management team.
§ Required data systems may include but are not limited to: the City’s Homeless Management Information System (HMIS), Goldie Health or a comparable solution to be utilized citywide for opioid outreach, PEP’s Electronic Health Record (EHR), and Maryland’s Health Information Exchange (HIE) known as Chesapeake Regional Information Sharing for Patients (CRISP).
§ Present timely reports for continuous quality improvement, population health, program strategic development and as required by contract or per partner organization request
§ Ensure service delivery, staff effort, and client outputs and outcomes are accurately and comprehensively represented in data collection practices and data reports.
- Utilize data and qualitative feedback in program improvement and strategic decision making, especially feedback from clients, particularly those who use drugs and those who are unhoused
- Improve community and public responses to homelessness, mental illness, addictions, and trauma by (professionally and with discretion) elevating the perspectives and experiences of program clients, providing relevant data, collaborating with partners, participating in workgroups, and facilitating staff/client engagement in system improvement activities when appropriate.
- Lead or participate in internal workgroups or committees to advance PEP’s mission
- Monitor program contract deliverables and provide timely updates on achievement, successes, and barriers.
- Maintain expertise in Housing First, Harm Reduction, Motivational Interviewing, Trauma/Resilience, Peer Support, Person-Centered Care Coordination/Case Management, Street Medicine, and other relevant models of care.
- Monitor the budget as well as billing and grant expenditures for the program, ensuring cost-effective and optimal allocation of funding and other resources for the benefit of clients.
- Ensure program environment, including assigned office(s), vehicle(s), and equipment are operating efficiently, clean, and well maintained.
- Research and develop opportunities for program and resource development, such as grants, donors, partnerships, and collaborative projects.

EDUCATION, WORK EXPERIENCE AND JOB REQUIREMENTS
· Education: Master’s degree in public health, social work, substance use, social science, psychology, or related field required.
· OJT: Three to six months training required for effective performance
· Job Requirements:
o Minimum of five years of management or leadership experience
o Excellent analytical, communication, writing and project management skills
o Experience with grants management or grants reporting
o Valid driver’s license and access to a personal vehicle
o Demonstrated ability to conceptualize complex initiatives and develop and implement plans to realize strategic objectives
o 3-5 years experience working with unhoused individuals
o 3-5 years experience working with individuals with substance use disorders
