Case Manager – Supported Living Services

Now, we’re looking for a Case Manager who’s ready to lead with heart and purpose.
What You'll Do
- Coordinate and oversee services for adults with developmental disabilities.
- Lead and mentor a team of Direct Support Professionals.
- Monitor daily/monthly documentation for accuracy and compliance.
- Facilitate meetings with individuals, staff, and service coordinators.
- Communicate regularly with Regional Center representatives.
- Ensure services meet high standards for quality, compliance, and person-centered care.
- Collaborate with leadership on innovation and continuous improvement.
- Build relationships with families and community partners.
- Regularly visit sites to support staff, recognize achievements, and maintain team morale.
- Step in when needed to ensure seamless care and service continuity.
- Education or experience:
- A Bachelor’s degree in Human Services, Psychology, Social Work, or a related field OR
- At least 2 years of leadership experience in human services (especially developmental disabilities).
- A compassionate, proactive leader who’s committed to inclusion, dignity, and growth.
- An effective communicator who builds strong teams and positive culture.
- Highly organized and reliable, with the flexibility to adapt to changing needs.
- Comfortable with compliance and quality assurance.
- Able to travel locally and hold a valid driver’s license (if applicable).
- Experience with adults or children with developmental disabilities is a plus.
- Bilingual skills are always a bonus!
