What are the responsibilities and job description for the Social Service Case Manager position at Pennyrile Area Development District?
The Pennyrile Area Development District (PeADD) has a job opening for a Social Service Case Manager in our Long-Term Services and Supports Department. Duties will include providing case management services to determine the client’s eligibility for in-home services. Provide home visits every other month and maintain necessary documentation.
Develop plan of care and ensure referrals are made for needed services. Compiling monthly and annual report information, educating clients regarding new projects, and maintaining contact with community organizations within the nine-county service area to increase awareness of the agency programs.
The qualified case manager shall meet one of the following qualifications:
- Bachelor’s degree in a health or human services field from an accredited college or university, with:
1. One (1) year experience in health or human services; or
2. The educational or experiential equivalent in the field of aging or disabilities.
- Associate degree in behavioral health, social science, or a closely related field and two (2) years of human service experience; or
- RN currently licensed as defined in KRS 314.011(10), and has at least two (2) years of experience as a professional nurse in the field of aging or disabilities; or
- A Master’s degree, in a health or human services field, from an accredited college or university can substitute for the required experience.
Pre-requisite job training and personal transportation is required to carry out contract functions. Excellent communication, organizational, and computer skills are a must.
