We are currently searching for a Care Navigator in our Outpatient clinic. Collaboration of systems and services is an important factor for increasing positive outcomes for individuals who are in treatment for opioid addiction. Care Navigators work directly with individuals with substance use disorder and provide support to bridge connections between treatment providers, health care professionals, insurance agencies and other services that may be beneficial to recovery.
Overcoming addiction is a multifaceted and individual process. Care Navigators work with individuals and service providers to connect clients to programs and timely assistance that will promote a recovery environment based on recommendations from treatment professionals. From a person's entry into the treatment process through various possible programs and supports a Care Navigator is available to encourage, connect and support treatment plans.
This position involves transporting clients to appointments in as needed. EOE.
