What are the responsibilities and job description for the Social Worker Skilled Nursing position at Penney Retirement Community?
General Summary: Responsible for planning, developing, organizing, implementing, evaluating, and directing social service programs of the skilled nursing facility.
Position Responsibilities:
- Responsible for providing services to meet the social and/or emotional needs that affect our residents’ ability to achieve their highest level of function.
- Participate in the development of the resident’s individualized comprehensive plan of care.
- Participate in QAPI and interdisciplinary meetings.
- Maintains regular social service progress notes including initial and quarterly assessments.
- Responsible for all aspects of discharge planning including coordinating discharge meetings, scheduling transportation, coordination of DME, and completing appropriate discharge paperwork.
- Serves as the Grievance Officer and Abuse Coordinator for the facility.
- Educates staff regarding resident psychosocial issues.
- Responsible for accurate and timely completion of PASRR
- Responsible for completion of Medicaid applications as needed.
- Provides crisis intervention, emotional support and counseling as needed to residents, families and staff.
- Assists with residents and family members with Advanced Directives that accurately reflect their wishes.
- Perform other duties that may be assigned by the Administrator.
EDUCATION REQUIREMENTS:
- Bachelor’s Degree or Master’s Degree in social work
REQUIRED EXPERIENCE:
- At least two years recent experience in a health care setting
- Geriatric experience preferred
- Must possess excellent communication and organization skills
- Relates positively to residents, families and staff members
- Basic computer knowledge and ability to learn computer software

Pay: $28.00 - $30.00 per hour
