The PT will deliver care in accordance with a physician’s plan, helping patients regain mobility and independence. Responsibilities Evaluate patients’ functional status and develop care plans Assess home environments and recommend modifications Provide direct physical therapy treatments Educate patients and caregivers on therapeutic techniques and equipment Monitor progress and adjust treatment plans Collaborate with physicians and interdisciplinary teams Document clinical notes and progress reports Participate in discharge planning Supervise Physical Therapy Assistants per policy and regulations Qualifications Active California Physical Therapy License Minimum 2 years of physical therapy experience (home health preferred) Strong communication and organizational skills Current CPR certification Valid driver’s license and insured, reliable vehicle Benefits Competitive compensation Health, dental, vision, life, and disability insurance 401(k) with company match Critical illness coverage Paid time off Employee Assistance Program (EAP) Seaport Scripps Home Health is an Equal Opportunity Employer.
