The Stroke Program Coordinator is a registered nurse, responsible for the coordination and management of the accredited Primary Stroke Center, oversees the care of stroke patients throughout Penn Medicine Princeton Health, with special focus on quality, patient safety and patient satisfaction. The Stroke Coordinator will work collaboratively with the Medical Director of the Stroke Program to ensure quality care for the stroke patient throughout the continuum. The Stroke Program/Regulatory Affairs Coordinator collaborates with the stroke leadership team members, to ensure comprehensive clinical treatment, decisions, and diagnoses for inpatients and will oversee medical record documentation to reflect best practice standards of care. The Stroke Program/Regulatory Affairs Coordinator will build relationships, support patients, and solve problems with the end goals being elimination of barriers to care, timely delivery of services and better patient outcomes. Additionally, the Stroke Program/Regulatory Affairs Coordinator, working under the direction of the Regulatory Affairs Manager, will assist the organization to maintain compliance with standards and regulations for the Center for Medicaid and Medicare Services, NJ Department of Health, The Joint Commission (TJC), and others regulatory bodies. Stroke Program/Regulatory Affairs Coordinator works collaboratively with organizational leaders and staff to identify and address performance and process improvements gaps in patient safety and quality of care. In addition, The Stroke Program/Regulatory Affairs Coordinator works with the Penn Medicine leaders across the system to align and standardize best practices in care across the campuses.

Summary:

  • The Stroke Coordinator is responsible for the coordination and management of the accredited Primary Stroke Center and oversees the care of stroke patients throughout Penn Medicine Princeton Health. The Stroke Coordinator works collaboratively with the Medical Director of the Stroke Program and with members of the stroke leadership team to ensure comprehensive clinical treatment, decisions, and diagnoses for inpatients. Also oversees medical record documentation to reflect best practice standards of care. The Stroke Coordinator establishes and maintains relationships, supports patients, and solves problems with the end goals being elimination of barriers to care, timely delivery of services and better patient outcomes. This position is responsible for supporting organization-wide performance improvement efforts including data collection, submission, analysis and interpretation as it relates to specific services and/or processes. This position supports departments/service areas to achieve improvement where opportunities have been identified. The performance improvement responsibilities support the organization to continuously meet regulatory and accrediting requirements including submission of quality metrics to respective organizations.

Responsibilities:

  • Supports and ensures Princeton Medical Center’s continued certification as a Joint Commission and State of NJ designated primary stroke center. Provides education, reporting and marketing for the Stroke Program and other assigned programs. Responsible for quality and regulatory compliance for the Stroke Program and other assigned programs; coordinates and delivers education and development opportunities for clinical staff as it relates to the Stroke Program and other assigned programs. Ensures quality and regulatory compliance for the Stroke Program and other assigned programs. Coordinates and delivers education programs and in-services for clinical staff as it relates to the Stroke Program or other programs. Supports the Peer Review Process through screening medical records and summarizing for Department Chairs. Collaborates with physicians, nursing, physical therapy, food service, marketing, pharmacy, and outreach programs to assist in coordinating a continuum of care for our patients. Analyzes data from a variety of sources on established metrics to identify opportunities for improvement and coordinates and/or collaborates with key stakeholders for improvement implementation. Performs data analysis and interpretation identifying trends in metrics from assigned departments/service area. Works in collaboration with marketing and outreach to conduct community education seminars in targeted outreach areas to increase stroke awareness and grow market share. Actively participates in local and enterprise governance structures/ committees Performs annual reviews and updates to Stroke Program and other assigned programs. Provides education regarding Performance improvement throughout the organization.

Credentials:

  • BLS/CPR, as a healthcare provider as per the American Heart Association
  • Registered Nurse - NJ (Required)
  • Stroke Certified Registered Nurse within 2 years of starting in the position

Education or Equivalent Experience:

  • BSN and 5 years of Clinical experience in Neuroscience Nursing or clinical quality / project management experience
  • Masters degree and 5 years of Regulatory Compliance and/or Patient Safety Experience preferred

Salary Range:

$86,306.69 - $137,585.76/Annually based on 40 hours per week
