Summary:
The outpatient advanced heart failure program is looking for a full-time social worker to join the team! The ideal candidate will have a minimum of 2 years of experience in a medical environment, bereavement, or chronic illness.
Responsibilities:
Provide a range of clinical services and case management to patients and their families coping with acute and chronic conditions in the Advanced Heart Failure Program.
Demonstrate thorough working knowledge of social work process, appropriate clinical interventions, community agencies, services, and resources available to assist patients and families with emotional and social difficulties related to change in patient’s health status.
Proficiency in crisis intervention and conflict resolution, understanding of financial assistance programs available to patients, competence in cultural sensitivity and inclusivity.
Excellent communication skills, flexibility, and ability to work in fast-paced environment.
Ability to work collaboratively as a member of a large multidisciplinary team.

Credentials:
- Licensed Social Worker (preferred)
- Education Specialization: Master's in Social Work
- Experience: 2 years of experience as a licensed social worker in a medical environment, bereavement, or chronic illness (preferred)
