Job Title: Patient Support Specialist

Entity: Hospital of the University of Pennsylvania Cedar (HUP Cedar)

Department: Emergency Psych Crisis (CRC)

Location: 501 S. 54th Street (54th & Cedar)

Hours: 1.0 FTE (40 hours per week), Day Shift

Summary:

  • The Patient Support Specialist is an individual with personal, lived experience in their own mental health and/or substance use recovery. The Patient Support Specialist will be embedded within the Crisis Response Center (CRC) and supports patients by offering a unique perspective and insight into the recovery process based on their own experience. The Patient Support Specialist is part of a multidisciplinary team at the CRC and is responsible for working in collaboration with other team members to support a patient's needs during the course of their treatment at the CRC.

Responsibilities:

  • PATIENT SUPPORT SPECIALIST DUTIES:

Patient Support Specialist will greet and support patients in the Crisis Response Center waiting area

Share information and patient needs (social service, health and medication) with appropriate interdisciplinary provider: PCP, social work, MAT provider, D&A provider, etc.

Serve as an essential link between patients and all other care providers

Monitors and evaluates the delivery of care through documentation and tracking of needs addressed, track outcomes and support data collection

Provide recovery education and support

Provide a model for both people in recovery and staff by demonstrating that recovery is possible.

Assist recovering persons to identify their personal interests, goals, strengths and weaknesses regarding recovery.

Actively identify and support linkages to community resources (communities of recovery, MAT, educational, vocational, social, cultural, spiritual resources, mutual self- help groups, professional services, etc.) that support the recovering person’s goals and interests. This will involve a collaborative effort including the recovering person, agency staff and other relevant stakeholders.

Support connections to community based, mutual self-help groups. Link individuals to appropriate professional resources. Provide vision-driven hope and encouragement for opportunities at varying levels of involvement in community based activities (e.g., work, school, relationships, physical activity, self-directed hobbies, etc.).

Credentials:

  • Certified Recovery Specialist (Required)
  • Must maintain valid CRS certification and remain in good standing valid during employment Must enroll in an approved CPS or CRS course within six (6) months of hire and receive PCB (PA Certification Board) certification as a CPS or CRS within one (1) year of hire

Education or Equivalent Experience:

  • H.S. Diploma/GED (Required)
  • Education Specialization: Equivalent Experience:
  • Associate of Arts or Science
  • Education Specialization: Behavioral Health, Nursing or related field of study required
  • Equivalent Experience: Prior behavioral health experience
  • Knowledge of behavioral health resources
