Position Description

Pennsylvania Hospital is seeking a Clinician to join our PHIICAPS Team at Hall Mercer. This is a full-time opportunity.

Summary:

  • In collaboration with Mental Health Worker team member, Clinician provides community-based therapeutic services and case management for children ages 4-19 with severe emotional disturbance (SED) who are at high risk of hospitalization, juvenile justice involvement and foster care placement. The Clinician also provides case management and therapeutic services for their caregivers and family members and collaborates with MHW in family therapy sessions. Clinicians are responsible for guiding the therapeutic treatment and writing all assessments, monthly treatment plans, and discharge summaries. Services include meeting and liaising with school personnel, mental health treaters, medical providers, DHS/CUA, and other providers. Travel to client homes, schools, hospitals, residential treatment facilities (RTFs) and other community sites is required. Clinicians also regularly provide crisis response services to support families in de-escalating and achieving safety. Each PHIICAPS team typically provides service to eight children and their families averaging of 4-5 hours working with each family every week. Paid after-hours on-call responsibilities are included in the PHIICAPS model. The standard schedule for Mental Health Workers and Clinicians is 10a-6p, M-F, but flexibility is required to meet clients’ scheduling and safety needs outside of those hours.

Responsibilities:

  • Complete assessments, tools and scales in-home with child and parents according to IICAPS treatment model. Create treatment-plans and discharge summaries and lead clinical direction of treatment.
  • Maintain clinical records reflective of all direct and collateral contact related to cases, as indicated by Hall-Mercer’s Policies and Procedures.
  • Support families in resolving crises and, when necessary, escalating a child to a higher level of care, including involuntary commitments (302s). Screen for suicidality and implement suicidality triage measures as needed. This includes paid evening/weekend on-call responsibilities a few weeks each year.
  • Research and refer clients to resources and services as needed, often using Family Support Funds to support stability and clinical outcomes.
  • Co-construct case formulation with clinician & family, participate in creation of treatment plans with family based on IICAPS model, and implement treatment plans
  • Complete utilization reviews and ancillary documentation as required by CBH for billing purposes
  • Schedule clients in PennChart and capture charges for services rendered for Accounting department
  • Participate in weekly rounds, supervision, training with Program Manager, Medical Director and other team members, as well as departmental and agency meetings.

Education or Equivalent Experience:

  • Master of Arts or Science (Required)
  • Education Specialization: Social Work, Counseling, Marriage & Family Therapy, or other related field Equivalent Experience: 1 year of experience in mental health or social services field working with children and families

Credentials:

  • Licensed Clinical SW - preferred
  • Licensed Social Worker - preferred
  • Licensed Prof Counselor - preferred
  • Marriage and Family Therapist - preferred
