Job Description
PURPOSE OF JOB : Assist with all aspects of delivery of care and support for patients in an outpatient setting including: Assisting physicians and Advance Practice Professioanls (NP, PA's)
PURPOSE OF JOB : Assist with all aspects of delivery of care and support for patients in an outpatient setting including: Assisting physicians and Advance Practice Professioanls (NP, PA's)
- Patient assessments and monitoring
- Patient education
- Triage nursing (telephonic and in person)
- Coumadin management
- Operation of equipment
- Chronic care management
- Risk stratifying of patients to identify high risk/ high cost patients and their medical needs
- Telephonic outreach to patients that have been discharged from the hospital, nursing home, emergency room and/or urgent care
- Other duties as requested
- Education:
- Graduate of an accredited School of Nursing. RN required.
- Current licensure in Commonwealth of Pennsylvania, Department of State Bureau of Occupational and Professional Affairs and successful completion of appropriate continuing education specific to job requirements
- Experience: Cardiology experience preferred
- Other Skills:
- Certified in BLS for Healthcare Provider
- Excellent interpersonal and team building skills
- eClinical Works experience preferred
- Knowledge of the following:
- State law requirements
- Certification requirements
- CME requirements
- Chart/medical record documentation
- Federal/state laws related to the release of health care information
- Computer functionality
- Nursing processes; health care system, structure and functions
- Skill in triaging patients, in-person and by phone
- Ability to calculate and administer drug dosages/injections and measure results
- Skill in initiating appropriate crisis interventions and emergency response
