Job Overview
Penn Medicine At Home is seeking an experienced Registered Nurse (RN) Case Manager to join our Home Health team serving the Princeton South region. This role is ideal for a nurse who thrives in a fast-paced, high-volume home care environment and is skilled in both patient care and care coordination.
Schedule
- Shift: Day Shift
- Hours: 8:00 AM – 4:30 PM
- Weekly Hours: 40 hours
- Assignment Duration: 13 weeks (April 15, 2026 – July 11, 2026)
Key Responsibilities
- Manage and coordinate care for home health patients across Plainsboro and West Windsor
- Conduct comprehensive patient assessments and develop individualized care plans
- Perform home visits (approximately 5–6 patients per day)
- Complete and maintain accurate documentation, including OASIS assessments
- Provide skilled nursing care and patient education
- Collaborate with physicians and interdisciplinary healthcare teams
- Monitor patient progress and adjust care plans as needed
Requirements
- Active Registered Nurse (RN) license
- Minimum 2 years of home health experience
- Case management experience (required)
- Strong experience with OASIS documentation
- Ability to manage a high patient volume and documentation workload
- Excellent communication and organizational skills
Preferred Qualifications
- Experience in wound care
- Background in high-volume home health settings
