Speech-Language Pathologist (SLP) – Home Health – Per Visit / PRN
Penn Home Health Care is seeking a Speech-Language Pathologist (SLP) to provide skilled speech therapy services in the home setting.
Service Areas:
Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, PA
Position Type:
Per Visit / PRN
Responsibilities:
• Perform speech therapy evaluations and reassessments.
• Develop individualized treatment plans.
• Address communication, cognitive, and swallowing disorders.
• Educate patients and caregivers.
• Collaborate with physicians and interdisciplinary team members.
• Complete timely and accurate documentation.
Qualifications:
• Current Pennsylvania SLP license.
• Certificate of Clinical Competence (CCC-SLP) preferred.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Home health experience preferred.
