Position Summary
The Personal Care Assistant/Home Health Aide (HHA) is a non-professional, trained caregiver responsible for providing personal care and related homemaking services to clients in their homes. The HHA supports clients in achieving and maintaining the highest possible level of independence in activities of daily living (ADLs). Under supervision and in accordance with PASSi policies and the client's individualized care plan, the HHA delivers compassionate, respectful, and culturally sensitive care that promotes safety, dignity, and well-being.
Position Duties And Responsibilities
The Personal Care Assistant/Home Health Aide (HHA) is a non-professional, trained caregiver responsible for providing personal care and related homemaking services to clients in their homes. The HHA supports clients in achieving and maintaining the highest possible level of independence in activities of daily living (ADLs). Under supervision and in accordance with PASSi policies and the client's individualized care plan, the HHA delivers compassionate, respectful, and culturally sensitive care that promotes safety, dignity, and well-being.
Position Duties And Responsibilities
- Assist clients with personal care needs, including oral hygiene, skin care, hair and nail care, and shaving male clients.
- Support clients with positioning and mobility to maintain proper body alignment and prevent skin breakdown.
- Assist with medications that are ordinarily self-administered, in accordance with the care plan and agency policy.
- Take and record vital signs (blood pressure, temperature, pulse, respiration) as required by the care plan.
- Promote client independence by assisting with and reinforcing activities of daily living (ADLs).
- Teach and support clients in the use of assistive devices such as canes, walkers, or adaptive utensils.
- Plan, shop for, and prepare nutritious meals, including special diets when required.
- Provide light housekeeping services related to client care.
- Promote mental alertness and emotional well-being through conversation and purposeful activities.
- Monitor, observe, and promptly report any changes in the client's condition, behavior, or appearance to the Supervising Nurse.
- Complete all required documentation accurately and on a daily basis in accordance with agency policies and procedures.
- Attend required online in-service trainings as scheduled.
- Employment is contingent upon a criminal background check free of prohibitive offenses.
- Must be legally authorized to work in the United States.
- High school diploma or GED preferred.
- Valid driver's license preferred.
- Bilingual Spanish applicants strongly desired
- Ability to follow oral and written instructions accurately.
- Ability to maintain confidentiality of client information in accordance with agency policies.
- Ability to use proper body mechanics to meet physical job demands safely.
