Peninsula Healthcare Connection is seeking a Harm Reduction Specialist with required certifications and experience to make $65-75000 annually as we try to change the experience of the unhoused and medically complex in Santa Clara County. Come work with a committed and passionate multidisciplinary community as part of our New Directions, Permanent Supportive Housing department. The position's schedule is Monday through Friday, 10:30am-7pm.

This position will have Alternative Work Week schedule that will include one weekend day, but will still have two days off in a row. There is a salary differential for Alternative Work Week schedules.
This position will:
- Utilizing a harm reduction approach, provide engagement, counseling and resource connection, including harm reduction supplies, to address behaviors that may pose risks such as drug and alcohol use, sexual practices, and self-injury
- Provide consultation, education, training, and support to multi-disciplinary supportive services staff for harm reduction informed services and interventions.
- In conjunction with other on-site supportive services providers, create and facilitate programming and groups that seek to increase individual and overall community health and safety.
- Assist permanent supportive housing residents to access services and supports that foster recovery, health and resilience, housing retention, and meaningful daily activity.
- In conjunction with multi-disciplinary team, provide incident response services utilizing harm reduction strategies or appropriate emergency response procedures.
Required Skills
- SA certification such as CADC strongly required, other SA certifications acceptable as well.
- Minimum two years’ experience providing direct services to vulnerable populations such as individuals experiencing homelessness, chronic medical conditions, substance use and/or mental health conditions.
- Must be familiar with and believe in a harm reduction philosophy, and able to communicate without judgment and shame.
- Demonstrated ability to develop collaborative partnerships with staff, clients, other professionals, and community members, including individuals from diverse cultural and socio-economic backgrounds.
- Ability to use knowledge of harm reduction strategies to create and facilitate programming and groups in a permanent supportive housing community.
