Position Summary
The Population Health RN supports individuals with chronic medical and behavioral health conditions by providing care coordination, education, and clinical support. This role works collaboratively with an interdisciplinary team to improve health outcomes, promote self-management, and support clients in living independently in the community.
Key Responsibilities
- Manage care for individuals with chronic health conditions through assessment, planning, and follow-up
- Conduct outreach and engage clients in services and care plans
- Provide patient education on condition management, medications, and warning signs
- Develop individualized care plans and support self-management goals
- Coordinate care with providers, health plans, and community resources
- Support transitions of care and participate in population health initiatives
- Document care accurately in EMR systems
- Participate in crisis intervention, screening, and stabilization services as needed
- Provide clinical services including medication administration, monitoring, and home visits (as applicable)
- Collaborate with interdisciplinary teams and support staff education
Qualifications
- Graduate of an accredited nursing program
- Current Washington State RN (or LPN) license
- Minimum 1–3 years of clinical experience preferred
- Valid driver’s license, insurance, and reliable transportation
- Ability to pass background check, drug screening, and meet immunization requirements
Preferred Experience
- Experience in behavioral health, population health, or chronic disease management
- Work with individuals with serious mental illness or complex care needs
Skills & Abilities
- Strong communication, care coordination, and patient education skills
- Ability to work independently and as part of a team
- Knowledge of medication management and clinical documentation
- Effective time management and problem-solving skills
Work Environment & Requirements
- Combination of office, community, and client-facing settings
- Frequent standing/walking and occasional lifting (up to 25 lbs)
- May include occasional evenings or weekends
- Exposure to clinical and behavioral health environments
