About the Role:
We are seeking an empathetic and highly organized Case Manager to join our dedicated team. This is a unique "temp-to-perm" opportunity designed as a working interview. You will start as a temporary employee for an initial 90-day evaluation period. Key Responsibilities:
- Manage a caseload of 17 clients, conducting regular assessments and developing personalized care plans.
- Coordinate with medical, social, and community resources to ensure holistic client support.
- Maintain accurate, up-to-date documentation and client files in compliance with agency standards.
- Advocate for clients and act as the primary point of contact for their needs.
Qualifications:
- Bachelor's degree in Social Work (BSW) or related human services field.
- At least 2 years of case management experience.
- Strong crisis intervention skills and knowledge of local Suffolk County resources.
- Exceptional communication and time-management abilities.
