Job Summary
PDHS is seekinga Clinical Behavioral Health Specialist (CBHS) to ensure continuity of care to pregnant and postpartum women, men, infants and families. The priority is to assist in the design of care plans and care coordination, develop in-service staff training, and advise on participant and community education to address disparities in perinatal outcomes towards healthy birth outcomes. The CBHS also, participates in and provides collaborative input in all QA/QI activities regarding overall care, reviews Healthy Start benchmarks of maternal health to ensure that the benchmarks are achieved or exceeded using QI/QA reviews, work plans, and staff enrichment activities. Furthermore, abides by local, state, and federal laws governing health and service information by maintaining confidentiality, security, and integrity of protected health information. The position is grant funded.
Duties and Responsibilities
· Facilitates the integration of care coordination services to pregnant, postpartum women, men as partners, infants, and family members
· Provide counseling, therapy, and/or behavioral interventions to support mental health and emotional well-being of mothers, fathers, and families
· Offer diagnosis, treatment planning, follow up, tracking and therapy for mental health conditions like depression, anxiety, and postpartum mood disorders
· Coordinates the development and implementation of a quality assurance and quality improvement plan to achieve Healthy Start benchmarks related to women’s health
· Assists in data collection, analysis, and tracking of federal performance measures; implement corrective measures for performance measures not achieved
· Coordinate and facilitate staff training, community education and stakeholder workshops on topics relevant to social determinates of health that impact infant and maternal health
· Maintains current knowledge on emerging models to address culturally competent service delivery
· Build relationships with stakeholders in maternal care, including local experts, representatives of clinical and behavioral health organizations spanning professional societies, advocacy groups, health plans, research institutions and government entities including the BOI, MMR Committee, etc.
Skills
Must have a thorough knowledge of service delivery systems; ability to interact effectively on an individual and group-based level, coordinate activities with individuals, families, and groups, and manage one’s time effectively, follow up and track service delivery activities. Must be able to develop working relationships with individuals, agencies, organizations, and groups in a collaborative way. Must communicate effectively, both written and verbally. Be proficient in the use of computers and related software and data input systems. Additionally, you must have a clean driving record with reliable transportation. You should be able to lift up to 35 pounds.

Pay: $60,000.00 - $70,000.00 per year
