Registered Nurse (RN) Case Manager – Pediatric Home Health
Location: Minnetonka, MN
Pediatech Nursing is seeking an experienced, compassionate, organized, and dedicated Registered Nurse (RN) Case Manager to join our growing team. This is a full-time, permanent opportunity with a pediatric home health agency serving children and young adults throughout the Twin Cities, St. Cloud, and select rural Minnesota communities.
As an RN Case Manager, you will play a vital role in coordinating exceptional patient care while providing clinical leadership and support to our field nursing staff. Position Responsibilities
Responsibilities include, but are not limited to:

Conduct in-home nursing assessments for pediatric and young adult clients.

Develop, implement, and update individualized plans of care.

Supervise and support field nurses to ensure high-quality patient care.

Facilitate orientation and onboarding for newly hired nurses.

Provide ongoing clinical training, education, and support to nursing staff.

Ensure compliance with agency policies, procedures, and regulatory requirements.

Recommend updates to clinical policies and procedures to maintain best practices and regulatory compliance.

Coordinate care with physicians, specialists, clinics, hospitals, durable medical equipment (DME) providers, and other members of the interdisciplinary care team.

Collaborate with fellow RN Case Managers and office staff to provide seamless care coordination and exceptional patient outcomes.
Qualifications
Experience
-
Minimum 3 years in Nursing
- 2 years in Pediatric nursing experience preferred.
- 2 years in Home Health Preferred
Education & Licensure

Bachelor of Science in Nursing (BSN) preferred. Associate Degree in Nursing (ADN) may be considered with Governing Body approval.

Current, unrestricted Registered Nurse (RN) license in the State of Minnesota.

Valid driver's license with a satisfactory driving record.
