Registered Nurse (RN) Case Manager – Pediatric Home Health

Location: Minnetonka, MN

Pediatech Nursing is seeking an experienced, compassionate, organized, and dedicated Registered Nurse (RN) Case Manager to join our growing team. This is a full-time, permanent opportunity with a pediatric home health agency serving children and young adults throughout the Twin Cities, St. Cloud, and select rural Minnesota communities.

As an RN Case Manager, you will play a vital role in coordinating exceptional patient care while providing clinical leadership and support to our field nursing staff. Position Responsibilities

Responsibilities include, but are not limited to:

• Conduct in-home nursing assessments for pediatric and young adult clients.

• Develop, implement, and update individualized plans of care.

• Supervise and support field nurses to ensure high-quality patient care.

• Facilitate orientation and onboarding for newly hired nurses.

• Provide ongoing clinical training, education, and support to nursing staff.

• Ensure compliance with agency policies, procedures, and regulatory requirements.

• Recommend updates to clinical policies and procedures to maintain best practices and regulatory compliance.

• Coordinate care with physicians, specialists, clinics, hospitals, durable medical equipment (DME) providers, and other members of the interdisciplinary care team.

• Collaborate with fellow RN Case Managers and office staff to provide seamless care coordination and exceptional patient outcomes.

Qualifications

Experience

• Minimum 3 years in Nursing

• 2 years in Pediatric nursing experience preferred.
• 2 years in Home Health Preferred

Education & Licensure

• Bachelor of Science in Nursing (BSN) preferred. Associate Degree in Nursing (ADN) may be considered with Governing Body approval.

• Current, unrestricted Registered Nurse (RN) license in the State of Minnesota.

• Valid driver's license with a satisfactory driving record.
