Salary: Starting at $80,000
Description:
The Wellness Nurse to monitor resident health, implement wellness plans, and coordinate with care teams to promote a safe and healthy environment.
Responsibilities:
- Assess and monitor resident health, including vital signs and chronic conditions
- Administer medications and treatments within scope of practice
- Collaborate with physicians, nurses, and care teams on wellness plans
- Educate residents and staff on health, nutrition, and preventive care
- Maintain accurate health records and document interventions
- Support resident engagement in wellness programs
Qualifications:
- Active LPN or RN license in Illinois
- Experience in assisted living, long-term care, or senior care (Required)
- Knowledge of geriatric care and wellness principles
- Strong communication, organizational, and teamwork skills
